This form is for graduate students requesting to enroll in an online course offered by the School of Professional Studies (SPS). This is NOT for Broadway-campus distance courses.

Important:

You must contact the appropriate office before the registration deadline if you have any hold on your student account.
Broadway Campus tuition rates/fees will be charged.
SPS may differ from your current program start, drop, and withdrawal dates.

We do not have access to register students after the registration deadline. If you exceed the course hour limit, you must submit an Academic Course Overload form to the Registrar's Office before the registration deadline.
