  • Instructions: Students wishing to change to a different graduate program should use this form.

    School of Professional Studies (SPS): students currently enrolled in an SPS graduate degree program typically must apply for admission through the UIW Graduate Admissions office. This form connects SPS with the other Program Director(s).

    UIW Health Professions: Graduate students who wish to change to a health profession program must follow the admissions application process described on the website of each health profession school. Professional students in a health profession program may use this form to connect with other graduate programs.

    Important: Requesting a change of academic program is an important decision that requires considerable thought. By submitting this form, you

    • acknowledge you are aware of the academic requirements for the new degree program;
    • understand this change may affect your intended graduation date;
    • understand that submission of this form does not guarantee that a change of program will be approved because some degrees have requirements (i.e., minimum GPA) that must be met to facilitate the change.

    Submission Timeline: This form should be submitted three weeks before the requested semester/term begins to avoid processing delays.
